Step 1: Under "Health Plan Name," write Texas Children's Health Plan as your 1^st choice for each recipient.

Step 2: Look through the Texas Children's Heath Plan directory and pick a Primary Care Provider. Write down your 1^st and 2^nd choice for doctors.

Step 3: Fill out the rest of the form. Sign and date it, then mail it in using your return envelope.
