How do I
choose Texas Children’s Health Plan?
After you receive your enrollment form, you will need to
write in Texas Children’s Health Plan as your health plan. If
you need help filling out the form call the STAR Help Line at

Texas Children’s Health Plan offers STAR in Brazoria, Fort
Bend, Galveston, Harris, Montgomery and Waller counties.

What if I have questions about my child’s health?
You should always ask your doctor about your child’s health.
If you don’t know who your doctor is then call the Member
Services Department at 1-866-959-2555. If it is after your
doctor’s office is closed and you don’t know what to do, you can
call our Nurse Help Line at
1-800-686-3831.
What if I want to change my doctor?
