Apply for CHIP coverage
You may apply for CHIP coverage if:
- You are a parent who lives with
your children who are 18 years or younger.
- You are a grandparent, relative
or other adult who lives with and takes care of a child.
- You are a teenager who lives on
your own.
Applying for CHIP coverage is quick
and easy. Simply
print an application and instructions, call CHIP at
1-877-543-7669 or Texas Children’s Health Plan at
1-800-990-8247. Once you have completed the application, mail it
to:
