letter of interest, your curriculum vitae, a list of your
hospital privileges and specialties to Texas Children's Health Plan. Submissions may

Liberty, Matagorda, Montgomery, Newton, Orange, Polk, San Jacinto,
Tyler, Waller, Walker and Wharton counties.
For STAR: Brazoria, Fort Bend,
Galveston, Harris, Montgomery and Waller counties.
