Your health is just as important as your baby’s. That’s why you must complete a postpartum visit with your provider 21 to 56 days after giving birth. When you do, we’ll send you a portable crib! A portable crib helps you make sure your baby plays and sleeps safely.
How do I claim my extra benefit?
You will receive a post card that you’ll need to take to your doctor. At the end of your postpartum visit, fill out the card and ask your doctor to sign verifying your visit. Then mail the post card, at no cost to you, to Texas Children’s Health Plan.
How will I receive it?
You will receive your extra benefit via U.S. Mail. If you are a patient at The Center for Children and Women, you will have the option to receive your extra benefit on-site, upon completion of your postpartum visit.
