Texas Children’s® Health Plan invites you to participate in our upcoming CME: Advancing Mental and Behavioral Health in Primary Care: Strategies for Diagnosis, Treatment, and Coordination. During this course, providers will develop evidence-based strategies and plans to help provide effective care coordination and behavioral health integration models as seen in primary care.
This is a free, live course for contracted providers of Texas Children’s® Health Plan provided by Texas Children’s Hospital®.
Learning Objectives:
- Develop evidence-based strategies and plans to effectively diagnose, treat, and manage mental and behavioral health conditions seen in primary care
- Prepare culturally competent and equitable approach to patient counseling and behavior modification strategies to reduce risk of complications and improve outcomes related to mental and behavioral health
- Establish pathways to provide effective care coordination and behavioral health integration models
- Identify and mitigate barriers related to post-hospitalization follow up after acute mental and behavioral health events
