Long-acting Reversible Contraception Products Abandoned Unit Return Forms

Date: December 23, 2025

Attention: Providers

Call to action: The purpose of this communication is to inform providers that the updated Organon product abandoned unit return form, along with other LARC-related manufacturer forms, are available on the Vendor Drug Program (VDP) website.

Background: Long-acting reversible contraception (LARC) products are available to clients through either Medicaid pharmacy or medical benefit. Manufacturers offer abandoned unit return programs allowing prescribers to return an abandoned LARC product with an abandoned unit return form.

How this impacts providers: The updated Organon product abandoned unit return form, along with other LARC-related manufacturer forms, are available on the VDP website, https://www.txvendordrug.com/resources/forms

Next step for Providers: Providers should access and utilize the LARC-related forms from the VDP website and share this information with their staff.
