Depending on your needs, rides may include wheelchair-lift-equipped vehicles, stretcher vans, minivans, or ambulatory vans. Please let SRH know what type of ride you need when scheduling.

UPDATED Medicaid Re-enrollment Application Assistance is Available

Call to action: Texas Children's Health Plan (TCHP) would like to remind providers that assistance is available to their patients who need help with their Medicaid Reenrollment Application. Providers should encourage patients with Texas Children’s Health Plan coverage to contact us for assistance and support.
In-person application assistance at provider offices In-office Application Assistance may be available to providers that are interested in supporting the reenrollment process for their patients, pending availability and office location. To request application assistance in your office from TCHP’s Application Assistance team, please fill out an Application Assistance site request form.
Community in-person application assistance is available now for your patients For your patients who have Texas Children’s Health Plan coverage and are having difficulty completing the renewal process, our Application Assistance team can help. They are available at multiple locations across the Houston area and will guide families through the application process for Medicaid and CHIP. The most current locations and schedule are on our website at texaschildrenshealthplan.org/application-assistance.

Families should bring the following documents with them to the application assistance location.
Child's Social Security Number (the actual card is not needed)
Child's U.S. Birth Certificate or proof of legal residency status
Family income within the last 30 days. Physical documentation is required to prove the family’s income. Acceptable documentation includes the following:
Letter from the employer, check Stubs, Most Recent Income Tax, W-2 Form, and Wage Forms (Self Employment H1049 or Employment Verification H1028).
