Changes to Vitamin B12 (J3420) Coverage in the TMPPM

Date: June 24, 2026

Attention: All Providers

Effective Date: August 1, 2026

Call to action: Texas Children’s Health Plan (TCHP) would like to inform providers that effective August 1, 2026, the Texas Medicaid & Healthcare Partnership (TMHP) will update section 6.142, “Vitamin B12 (Cyanocobalamin) Injections,” in the Texas Medicaid Provider Procedures Manual (TMPPM), Outpatient Drug Services Handbook. This update removes diagnosis restrictions and indications for procedure code J3420.

Vitamin B12 injections (procedure code J3420) will continue to be a benefit of Texas Medicaid and should only be considered for patients with conditions that are refractory to, or have contraindication to, oral therapy.

Next step for providers: Providers should share this information with their staff.
