NEW PEMS Enrollment Requirement: Physical Therapists Must Attest to the Location Where Services Are Provided

Date: February 3,2026

Attention: Physical Therapists

Effective date: February 16, 2026

Call to action: Texas Children’s Health Plan (TCHP) would like to inform all physical therapists of a NEW PEMS enrollment requirement. Effective February 16, 2026, you will be required to attest to the physical location where services will be provided as part of your PEMS Texas Medicaid enrollment or reenrollment process. This attestation helps the Texas Health and Human Services Commission (HHSC) determine whether a site visit is required for your enrollment.

How this impacts providers:Site Visit Requirements

Site visits are mandatory when a physical therapist:

  • Provides services at a designated office location, and
  • Provides services in clients’ homes

Site visits are not required when a physical therapist provides services in:

  • An institutional setting
  • A school
  • A location directed by the client

The requirements will not affect in-flight applications that were submitted before February 16, 2026. Providers that submit an enrollment application on or after February 16, 2026, must attach a completed attestation form to the application.

Next step for Providers: Providers should share this communication with their staff and follow the guidance offered in this alert and if needed, seek additional information from the resources named below.

The Physical Therapy (PT) Attestation Form Regarding Location of Services for Texas Medicaid will be stored in the Provider Enrollment forms section of tmhp.com.
