How do I claim my card?

• Register and attend a virtual Member Advisory Group meeting.
• Once you have joined the online meeting, submit your name in the meeting chat.

Who is eligible?

CHIP, STAR, and STAR Kids members who attend the Member Advisory Group meetings during January 1, 2025 and December 31, 2025.
