Diabetes Awareness and Wellness Network (DAWN) Center

TCHP reserves the right to update and/or change this information without prior notice due to the evolving nature of the COVID-19 event.Call to action: Texas Children’s Health Plan (TCHP) is happy to announce that we have partnered with the Diabetes Awareness and Wellness Network (DAWN) Center to serve as a viable resource for our adult diabetic members to help in their management of this chronic condition. The DAWN Center is a program within the Houston Health Department that provides diabetes self–management education and support for adults diagnosed with Type 2 diabetes or pre-diabetes.
How this impacts providers: The DAWN Center offers the following resources to members:
Online individual and group classes offered in both English and Spanish
Virtual cooking demonstrations
Diabetes prevention and diabetes self-management courses
Physical activity classes and virtual walking clubs and fitness challenges
Collaboration with local community partners to provide members with the following
Kidney disease prevention education
Foot care education
Medication management
Next steps for providers: Providers are encouraged to refer their adult TCHP members to the DAWN Center that would benefit from their resources. Membership is required and the requirements are minimal. Members must be eighteen (18) years old and have type 2 diabetes, prediabetes, or at risk for these.
