Caring for our Young Members
Join Our Case Management Program
The exclusive Case Management program by Texas Children's Health Plan is a benefit offered to help children birth through 20 years of age with a health condition (illness, injury, or disability). Our dedicated case managers will help you gain access to needed medical, social, educational, and other services.
The available case management programs include:
- An asthma program that teaches you how to control your or your child’s asthma.
- A diabetes program.
- A weight management program.
- A program to help manage the child’s behavioral and emotional changes.
- And more.

Extra Support for Special Health Needs
We offer extra support and education at no cost for special health needs such as asthma, autism, diabetes, mental health, weight management, and more. Our case managers will offer personal help, manage your treatment plan, and set goals for your health.

Healthy Rewards Program
