Physical activity can improve mental health, help manage weight, reduce the risk of disease, strengthen bones and muscles, and more. Texas Children’s Health Plan wants to help cover the membership fee of the fitness center that you join!
Reward*:
$50 reward card for members ages 21 and up who join and pay for a fitness center membership. Must provide a paid invoice. Limited to one per member, per reward year. A paid invoice will be required when you submit your request form for this reward. Failure to provide will result in your request being denied.
How do I claim my extra benefit?
Log into your MyChart account. New to MyChart? Set up an account here.
Under “Menu”, go to “Resources”
Click on "Healthy Rewards Program" and complete the request form for this reward.
