Physical activity can improve mental health, help manage weight, reduce the risk of disease, strengthen bones and muscles, and more. Texas Children’s Health Plan wants to help cover the membership fee of the fitness center that you join!

Reward card for up to $50 when you sign up for a membership to a fitness center.
How do I claim my extra benefit?
Click on the “Redeem” button below to enter your information and submit proof of payment. Texas Children’s Health Plan will confirm your eligibility and you will receive your reward card in the mail.
