Description:
Whether you are getting ready for sports, school or camp, a physical exam is the first step in making sure you are ready for action!
Reward:
One yearly sports or school physical exam at no cost with your primary care provider.
How do I claim my extra benefit?
Schedule an appointment with your primary care provider and bring the camp or school physical form to be completed during the visit.
Who is eligible?
CHIP, STAR, and STAR Kids members 5 to 19 years old who have completed a Well-Child Checkup in the last 12 months.
What service area is this benefit available in?
Harris, Jefferson, and Northeast service areas.
