Healthy Rewards/Value Added Services for 2023-2024

Call to action: Texas Children’s Health Plan (TCHP) is proud to present our Healthy Rewards Program/Value Added Services (VAS) program effective September 1, 2023 through August 31, 2024. These rewards and services are available in each line of business CHIP, STAR, and STAR Kids and Service Delivery Areas; Harris, Jefferson and North East (limitations may apply).

Member favorites, such as the Prenatal and Postpartum Visit Rewards, Portable Crib/Playpen, Diabetes Management Rewards and Well-Child Visit Rewards are returning, along with a new benefit for our STAR members age 21 and up to help cover the cost of a fitness center membership.

For the first time ever, providers can request a reward on behalf of a member. If you are treating a CHIP-Perinate covered member with a diagnosis of hypertension during their pregnancy, you can request to have a blood pressure monitor mailed to their home. At this time, this is the only reward that can be requested by a provider.

Extra benefits are available for the following categories:

Healthy Pregnancy	Portable Crib/Playpen Baby Care and Birth Classes – InJoy (online classes or mailed books) Prenatal Reward Card Receive a $25 reward card for completing at least one prenatal visit during pregnancy Postpartum Reward Card Receive a $25 reward card for completing at least one postpartum visit within 42 days of giving birth Blood Pressure Monitor (NEW – CHIP-Perinate only, Harris and Jefferson County)
Health and Wellness	Health Education Special Events Vision Benefit Diabetes Management Reward NEW lower age limit – receive a $50 reward for completing an HbA1C test for members 10 and up Receive $25 reward card for completing diabetic eye exam once per year Well-Child Checkup Reward (0-15mo) Receive a $50 reward card for completing 3 well-child checkups by the age of 15 months Receive an additional $100 reward card when completing 6 well-child checkups by the age of 15 months Young Adult Wellness Visit Reward (18-21yrs) Receive a $25 reward card for completing a yearly wellness visit Cervical Cancer Reward Receive a $25 reward card for completing a cervical cancer screening during an annual well-woman exam. STAR only, age 21 and up Mental Health Follow-Up Visit Reward Receive a $25 reward card for completing a mental health follow-up visit within 7 days after discharge from a mental health hospital or facility Health Risk Assessment Reward NEW - receive a $10 reward card for completing the Health Risk Assessment within 90 days of becoming a NEW member

How this impacts providers: Providers should encourage members to take advantage of the Healthy Rewards Program benefits/VAS that are applicable and helpful for them.

Healthmine – Healthy Connections (VAS reward card fulfillment)

TCHP is continuing our partnership with Healthmine, branded Healthy Connections, for our members, for our VAS reward card fulfillment. Healthmine provides an online, interactive portal where members can actively participate in tracking their health actions and fulfill VAS gift cards. Please note, only the Prenatal, Postpartum, Cervical Cancer, Diabetes Management, Well-Child Check, HRA completion and Young Adult Wellness Visit rewards are fulfilled by Healthmine.

Members can request their reward and create their Healthmine (Healthy Connections) account a few ways:

For members age 18 and up, call Healthmine at 866-475-1619 (TTY711). Healthmine customer service agents can help members create their account and intake member VAS requests
For membersunder the age of 18, they must create their account online by either:
1. Visiting healthyconnections.healthmine.com
2. Logging into their MyChart account (under “Menu”, head to “Resources” and click on “Healthy Connections” to sign up)

Please encourage members to create their Healthy Connections account (if not already done so) as this will be the only way members can request and redeem these rewards
