Healthy Rewards/Value Added Services for 2025-2026

Call to action: Texas Children’s Health Plan (TCHP) is proud to present our Healthy Rewards Program/Value Added Services (VAS) program effective September 1, 2025 through August 31, 2026. These rewards and services are available in each line of business CHIP, STAR, and STAR Kids and Service Delivery Areas; Harris, Jefferson and North East (limitations may apply).

NEW Texas Children’s Health Plan MyChart for Reward Requests
TCHP has made it easier than ever for members to request rewards through the new TCHP MyChart platform. Members are required to create and/or login to their MyChart to request the rewards request form.

TCHP MyChart member instructions:

Log into your MyChart account. New to MyChart? Set up an account here
Under “Menu” go to “Resources”
Click on "Healthy Rewards Program" and complete the request form for this reward.

Please encourage members to create their TCHP MyChart account (if not already done so) as this will be the only way members can request and redeem these rewards. Please note that TCH and TCHP MyChart have two separate login request processes. If a TCH patient has MyChart, but would like to add TCHP MyChart features – they need to follow the instructions above.

Member favorites, such as the Prenatal and Postpartum Visit Rewards, Portable Crib/Playpen, Diabetes Screening Rewards and Well-Child Visit Rewards are returning, along with a new benefit for our members who screened for Chlamydia. Please review the chart below for a full list of all rewards including updates to existing and new rewards.

Extra benefits are available for the following categories:

Healthy Pregnancy	Portable Crib/Playpen UPDATE – members must first complete at least one online InJoy Baby Care and Birth Classes Flu Prevention Reward Receive a $25 reward card when pregnant moms get their flu vaccine Baby Care and Birth Classes – InJoy (online classes or mailed books) Prenatal Reward Card UPDATE - Receive a $25 reward card for completing at least one prenatal visit during the first trimester of pregnant or within 42 days of enrollment with TCHP Postpartum Reward Card Receive a $25 reward card for completing at least one postpartum visit within 84 days of giving birth Blood Pressure Monitor (CHIP-Perinate only, can be requested by Providers)
Health and Wellness	Health Education Special Events Eyewear Support Diabetes Screening Reward UPDATE - Receive a $25 reward for completing an HbA1C test for members 10 and up once per year for members age 18 and up Receive $25 reward card for completing diabetic eye exam once per year NEW - Receive $25 reward card for completing a diabetic kidney function test exam once per year for members age 18 and up Well-Child Checkup Reward (0-30mo) Receive a $100 reward card for completing 6 well-child checkups by the age of 15 months Receive an additional $50 reward card when completing 2 additional well-child checkups between the age of 15 to 30 months Young Adult Wellness Visit Reward (16-21yrs) Receive a $25 reward card for completing a yearly wellness visit Cervical Cancer Reward (STAR only, age 21 and up) Receive a $25 reward card for completing a cervical cancer screening during an annual well-woman exam Mental Health Follow-Up Visit Reward Receive a $25 reward card for completing a mental health follow-up visit within 7 days after discharge from a mental health hospital or facility (the only auto fulfilled reward) Health Risk Assessment Completion Reward (CHIP and STAR Only) Receive a $10 reward card for completing the Health Risk Assessment within 90 days of becoming a NEW member Asthma Education Reward Members with a high risk Asthma diagnosis who enroll in and complete a 90-day case management program are eligible to receive a $25 reward card ADHD Management Reward Members age 6-12 who are newly diagnosed with ADHD, are prescribed and pickup their ADHD medication and complete a 30 day follow-up visit after the initial diagnosis are eligible to receive a $25 reward card NEW - Chlamydia Screening Reward (16-24yrs) Receive a $25 reward card for completing a Chlamydia screening once per year NEW - Childhood Flu Immunization Rewards (0-2yrs) Receive a $25 reward card for receiving a flu vaccine (ages 0-2).

Extra benefits valid during the eligible state fiscal year (September 1, 2025 to August 31, 2026). Must be an active Texas Children's Health Plan member at time of verification to receive extra benefit. Unless otherwise stated, reward must be redeemed in the current eligible year. Purchase of alcohol, tobacco, illegal drugs, or firearms is prohibited with the reward cards. Unless otherwise stated, rewards are limited to one member per eligible year. TCHP is not responsible for lost or stolen rewards. Depending on the benefit, the reward can be requested up to 60 days after the end of the eligible year. Allow up to 90 days for delivery of the reward.}

As a reminder, if you are treating a CHIP-Perinate covered member with a diagnosis of hypertension during their pregnancy, you can request to have a blood pressure monitor mailed to their home. At this time, this is the only reward that can be requested by a provider.
