Well-Child Checkups Reward

Description:

Well-Child Checkups are scheduled visits with your baby's primary care provider that track their growth and health. They may include physical exams, vaccinations, blood tests, and more. Texas Children’s Health Plan wants to reward you for staying on top of your baby’s health!

Reward*:

$100 reward card for:

• Members (age 0-15 months) who complete 6 Well-Child Checkups. Limit one per member, per reward year. Sixth visit must occur during eligible year.

$50 reward card for:

• Members (ages 15-30 months) who complete 2 Well-Child Checkups. Limit one per member, per reward year. 8th visit must occur during eligible year.

How do I claim my extra benefit?

Follow this process after completing at least six Well-Child Checkups by the age of 30 months:

1. Log into your MyChart account. New to MyChart? Set up an account here
2. Under “Menu”, go to “Resources”
3. Click on "Healthy Rewards Program" and complete the request form for this reward.

* This reward can be requested up to 30 days after the end of the eligible year (September 1, 2026–August 31, 2027).

Who is eligible?

CHIP, STAR, and STAR Kids members up to 0-15 months who complete six Well-Child Checkups.

CHIP, STAR, and STAR Kids members up to 15-30 months who complete two Well-Child Checkups. 

What service area is this benefit available in?

Harris and Jefferson service areas.
