SafeRide Health (SRH) is the new provider for all NEMT rides to doctor appointments and pharmacy visits.

Depending on your needs, rides may include wheelchair-lift-equipped vehicles, stretcher vans, minivans, or ambulatory vans. Please let SRH know what type of ride you need when scheduling.

SafeRide Health (SRH) es el nuevo proveedor de todos los servicios de transporte médico que no son de emergencia (NEMT, por sus siglas en inglés) hacia consultas médicas y farmacias.

Childhood Flu Immunization Reward

Description:

Childhood vaccinations save an estimated four million lives worldwide every year. The flu vaccine is safe and protects your child from getting sick with the flu.

Reward:

$25 reward card when your child (ages 0-2) gets a flu vaccine.

How do I claim my extra benefit?

Follow this process after your child (ages 0-2) gets a flu vaccine:

  1. Log into your MyChart account. New to MyChart? Set up an account here.
  2. Under “Menu”, go to “Resources”
  3. Click on "Healthy Rewards Program" and complete the request form for this reward.
  4. Click here to view a step-by-step guide on how to request your reward in MyChart.

*This reward can be requested up to 30 days after the end of the eligible year (September 1, 2025–August 31, 2026).

Who is eligible?

CHIP, STAR, and STAR Kids members age 0-2 who receive a flu vaccine.

What service area is this benefit available in?

Harris, Jefferson, and Northeast service areas.
