Chlamydia Screening Reward

Description

As your teen grows up, it’s important they learn how to protect their sexual health, even if they are not sexually active. Some STIs, like Chlamydia, may not cause symptoms but still cause serious health problems if they are not treated.

Reward:

$25 reward card when your child (ages 16-24) completes a Chlamydia screening.

How do I claim my extra benefit?

Follow this process after your teen (ages 16-24) completes a Chlamydia screening:

• Log into your MyChart account. New to MyChart? Set up an account here.
• Under “Menu”, go to “Resources”
• Click on "Healthy Rewards Program" and complete the request form for this reward.
• Click here to view a step-by-step guide on how to request your reward in MyChart.

*This reward can be requested up to 30 days after the end of the eligible year (September 1, 2026–August 31, 2027).

Who is eligible?

CHIP, STAR, and STAR Kids members (ages 16-24) who complete a Chlamydia screening.

What service area is the benefit available in?

Harris, Jefferson, and Northeast service areas.
