Starting December 15, 2025, SafeRide Health will become the new provider for all member rides to doctor appointments and pharmacy visits. After this date, Texas Children’s Health Plan will no longer use MTM for Non Emergency Medical Transportation (NEMT) services.

Asthma Education Reward

Description: 

Texas Children’s Health Plan rewards you for taking steps to manage your high-risk asthma and keeping track of your health! 

Reward*: 

$25 reward card for members with a new high-risk asthma diagnosis who enroll in and complete the 90-day asthma Case Management program. 

How do I claim my extra benefit? 

Members with asthma who are identified as high risk, will be contacted by their case manager to discuss enrollment in the 90-day disease management program. 

Once you have completed the program, you will be sent a $25 reward card via mail.

If you have questions about this reward or if you are eligible, please contact Member Services or your Case Manager. 

Who is eligible? 

CHIP, STAR, and STAR Kids members with a high-risk asthma diagnosis who enroll in and complete the 90-day asthma Case Management program.

*This reward can be requested up to 90 days after the end of the eligible year (September 1, 2025 to August 31, 2026). 

What service area is this benefit available in? 

Harris, Jefferson, and Northeast service areas.
