Reward*:
$10 reward card for eligible new members who complete the Health Risk Assessment within 90 days of enrollment.
How do I claim my extra benefit?
Follow this process after completing your Health Risk Assessment:
- Log into your MyChart account. New to MyChart? Set up an account here
- Under “Menu”, go to “Resources”
- Click on "Healthy Rewards Program" and complete the request form for this reward.
- Click here to view a step-by-step guide on how to request your reward in MyChart.
* This reward can be requested up to 30 days after the end of the eligible year (September 1, 2025–August 31, 2026).
Who is eligible?
New CHIP and STAR members who complete the Health Risk Assessment within 90 days of enrollment.
What service area is this benefit available in?
Harris, Jefferson, and Northeast service areas.
