Welcome new members! Fill out our Health Risk Assessment form so we can better understand your health care needs. If you have not completed the form yet, please click here.
Reward*:
$10 reward card for eligible new members who complete the Health Risk Assessment Risk or Health Needs Assessment within 90 days of enrollment.
How do I claim my extra benefit?
Follow this process after completing your Health Risk Assessment:
Log into your MyChart account. New to MyChart? Set up an account here
Under “Menu”, go to “Resources”
Click on "Healthy Rewards Program" and complete the request form for this reward.

* This reward can be requested up to 90 days after the end of the eligible year (September 1, 2026–August 31, 2027).
Who is eligible?
New CHIP and STAR members who complete the Health Risk Assessment within 90 days of enrollment.
