$10 reward card for eligible new members who complete the Health Risk Assessment within 90 days of enrollment.

How do I claim my extra benefit?

Follow this process after completing your Health Risk Assessment:

1. Log into your MyChart account. New to MyChart? Set up an account here
2. Under “Menu”, go to “Resources”
3. Click on “Healthy Connections” and sign up to redeem your reward card and access your balance**

Who is eligible?

New CHIP, STAR, and STAR Kids members who complete the Health Risk Assessment within 90 days of enrollment.

What service area is this benefit available in?

*This reward can be requested up to 30 days after the end of the eligible year (September 1, 2023 – August 31, 2024).

** It may take up to 90 days to view your rewards in your Healthy Connections account.
