Diabetes Screening Reward

Description:

Texas Children’s Health Plan rewards you for taking steps to manage your diabetes and keeping track of your health!

Reward*:

  • $25 reward card for members ages 18 and up who complete a diabetic (retinal or dilated) eye exam once per year
  • $50 reward card for members ages 10 and up who complete an HbA1c blood test every six months

How do I claim my extra benefit?

Follow this process after completing each diabetic test eligible for the reward:

  1. Log into your MyChart account. New to MyChart? Set up an account here
  2. Under “Menu”, go to “Resources”
  3. Click on "Healthy Rewards Program" and complete the request form for this reward.

Who is eligible?

CHIP, STAR, and STAR Kids members who have type 1 or type 2 diabetes that complete a diabetic (retinal or dilated) eye exam (ages 18 and up) and/or an HbA1c blood test (ages 10 and up).

What service area is this benefit available in?

Harris, Jefferson, and Northeast service areas.

*These rewards can be requested up to 30 days after the end of the eligible year (September 1, 2024 – August 31, 2025).
