Diabetes Management Reward
Description:
Texas Children’s Health Plan rewards you for taking steps to manage your diabetes and keeping track of your health!
Reward*:
$25 reward card for:
- Members (ages 18 and older) who complete a diabetic (retinal or dilated) eye exam once per year.
- Members (ages 10 and older) who complete an HbA1c blood test.
- Members (ages 18 and older) who complete a diabetic kidney function evaluation.
How do I claim my extra benefit?
Follow this process after completing each diabetic test eligible for the reward:
- Log into your MyChart account. New to MyChart? Set up an account here
- Under “Menu”, go to “Resources”
- Click on "Healthy Rewards Program" and complete the request form for this reward.

* This reward can be requested up to 30 days after the end of the eligible year (September 1, 2025–August 31, 2026).
Who is eligible?
CHIP, STAR, and STAR Kids members who have type 1 or type 2 diabetes that complete one or more of the following:
- A diabetic (retinal or dilated) eye exam (ages 18 and up).
- An HbA1c blood test (ages 10 and up).
- A diabetic kidney function evaluation (ages 18 or older).
What service area is this benefit available in?
Harris, Jefferson, and Northeast service areas.
