Blood Pressure Monitor (CHIP-P only)

Description:

If you are diagnosed with high blood pressure during your pregnancy or at the time of delivery, both you and your baby may be at risk for complications. Receive a blood pressure monitor at no cost so you can stay on top of your health and help your chances of living a heart-healthy lifestyle!

Reward:

A blood pressure monitor cuff at no cost.

How do I claim my extra benefit?

  1. Log into your MyChart account. New to MyChart? Set up an account here.
  2. Under “Menu”, go to “Resources”
  3. Click on "Healthy Rewards Program" and complete the request form for this reward.  Who is eligible for this benefit?

The blood pressure monitor can be requested up to 30 days after the end of the eligible year (September 1, 2024 – August 31, 2025).

Who is eligible?

CHIP-Perinate members who have been diagnosed with high blood pressure during their pregnancy or at the time of delivery.

What service area is this benefit available in?

Harris and Jefferson service areas.
