SafeRide Health (SRH) is the new provider for all NEMT rides to doctor appointments and pharmacy visits.

Depending on your needs, rides may include wheelchair-lift-equipped vehicles, stretcher vans, minivans, or ambulatory vans. Please let SRH know what type of ride you need when scheduling.

SafeRide Health (SRH) es el nuevo proveedor de todos los servicios de transporte médico que no son de emergencia (NEMT, por sus siglas en inglés) hacia consultas médicas y farmacias.

If you are diagnosed with high blood pressure during your pregnancy or at the time of delivery, both you and your baby may be at risk for complications. Get a free blood pressure monitor at no cost so you can stay on top of your health and help your chances of living a heart-healthy lifestyle!

How do I claim my extra benefit?

  1. Log into your MyChart account. New to MyChart? Set up an account here.
  2. Under “Menu”, go to “Resources”
  3. Click on "Healthy Rewards Program" and complete the request form for this reward.
  4. Click here to view a step-by-step guide on how to request your reward in MyChart.

The blood pressure monitor can be requested up to 30 days after the end of the eligible year (September 1, 2025 to August 31, 2026).

Who is eligible?

CHIP-Perinate members who have been diagnosed with high blood pressure during their pregnancy or at the time of delivery.

What service area is this benefit available in?

Harris and Jefferson service areas.
