Screening and Assessment Instrument (SAI) Completion Reward

Welcome new members! Enroll in our Case Management Program and complete our Screening and Assessment Instrument (SAI) form so we can better understand your medical, behavioral and functional needs. Please reach out to your Case Manager if you have not completed this form or have questions. 

Reward*: 

$50 reward card for eligible new members who enroll in the Case Management Program and complete the Screening and Assessment Instrument (SAI) within 90 days of enrollment. 

How do I claim my extra benefit? 

Follow this process after completing the SAI: 

1. Log into your MyChart account. New to MyChart? Set up an account here
2. Under “Menu”, go to “Resources”
3. Click on "Healthy Rewards Program" and complete the request form for this reward.

Who is eligible? 

New STAR Kids members who enroll in the Case Management Program and complete the Screening and Assessment Instrument (SAI) within 90 days of enrollment. 

What service area is this benefit available in? 

Harris, Jefferson, and Northeast service areas. 

*This reward can be requested up to 30 days after the end of the eligible year (September 1, 2024 – August 31, 2025).
